I've got some mildew on my throwable cushions that I'd like to get rid of. I've tried Simple Green and OxyClean, both have almost zero effect. I'm thinking Clorox in solution but wondering at what ratio as I don't want to damage the cushions since they're safety devices.
Does anyone have a no-fail solution?

What are the cushions covered with? If vinyl, give them a shot of Tub and Tile Cleaner and then rinse it off quickly. If nylon fabric, a Clorox solution won't hurt it. (But keep Clorox away from Dacron.)
We used Tylex this past weekend to clean the mildew off of the canvas and the bottom of seat cushions on my wifes Bayliner. It did a great job with little or no scrubbing required.
I also used it on the inside of my hull last year. Same result.

On vinyl, rinse quickly and thoroughly. On fabric, I wouldn't use it--it'll work, but it'll probably reduce the life of the fabric.
Well, I went the Clorox route, but it took a really long time. I followed the instructions on the bottle to "get rid of mold & mildew" at 3/4 cup / gallon of water. While I was doing other things on the boat, I'd give them all a good spray, work for a while, rinse the solution off, and then reapply. I probably did this ten times easily, but eventually the greatest majority of the mildew went away. I let them dry in the sun and if nothing else, they smell nice & clean now.
We lived on boat in the tropics and had plenty of mildew. What worked for us was a solution of vinegar and water...it was easy to find in far away places and was cheap...Good luck
