Courtesy flags when you clear customs in a particular country, club burgees, quarantine flags, skipper onboard flags etc. all can go from the starboard spreader.
The locations for US flags in nautical instances arise out of visibility in most instances. While under sail, the flag would not be able to be seen underneath the sails. Another instance where the flag isn't above everything would be the Gaff Rig - also seen as the flag pole in many yacht clubs. I once had a burgee hanging from the spreader and when I was on certain points of sail, the burgee would slap the mains'l about once a second. After about twenty minutes of fwap...fwap...fwap...fwap, I took it down.
